A 91-year-old a resident of Residential Care Facility for the elderly had a fall, striking his head and body and sustaining multiple left-sided rib fractures. Within a few days of first fall, he suffered two additional falls with personnel failing to notify his care-giver and personal representative.  His injuries included a displaced, fractured left hip. He was found in soiled diaper and great deal of pain prior to being transferred to ER by ambulance.  Case settled for $725,000
